Healthy and Home Link Worker – Part-Time 28hrs
Salary £25,000 (pro-rata £18,919)
Fixed term post until December 2023
Working with the Hospital Discharge Team at Warrington and Halton Hospital Trust, this is an exciting opportunity for the right person. We are looking for a self-starter who can work with confidence under their own initiative within a team environment and who is committed to supporting people to maintain their independence, good wellbeing and promote self-care.
The successful candidate will have experience of working in a frontline support role in the voluntary, health or public sector, delivering services in a client focused way, empowering and supporting people.
